Conduct

Aggression in our practice is sadly becoming increasingly common. We do not tolerate any physical or verbal aggression towards our staff. These incidents will be documented and discussed internally. We will take the appropriate measures, in accordance with the guidelines of the Dutch Doctor Federation KNMG. If necessary we will ask to leave the pratice

No-show policy

If you don't show up to your appointment without cancelling we will unfortunately have to bill you. By law we can bill you for our lost time: Consult GP, Nurse Practitioner of assistent : 25 euro- 50 euro's depending on the sprt of appointment. Understandable reasons may occur and we will take this into account. Cancel your appointment at least 24 hours in advance (business days only) - via the e-portal. ( Please note: Cancelling for monday means doing this on the latest on friday)

Minors

Minors below the age of 16 must have at least one parent/caretaker registered as our patient. This parent/caretaker will sign for the minor. If there is a divorce with shared parental custody, both parents must sign for the registration of the child. Patients 16 or older must sign themselves, under the Protection Of Personal Information Act (Wet bescherming persoonsgegevens)

De-registration

If you move and change physician let us know. We will transfer your medical files to your new GP as soon as we get the transfer request. Without your permission, we are not allowed to transfer your medical files to avoid losing them.

Mutation of Contact details

Please inform us if you change your address, telephone number, email, insurance. We need your recent contact details in case we need to contact you.
